STEP 1: Apply to the Program OakGov.com/IAmPontiac
Completing the online application is the first step to the enrollment process. You will be contacted directly by a case manager regarding our upcoming enrollment Sessions to find one that you are able to attend.
STEP 2: Gather Required Documentation
BRING TO ENROLLMENT SESSION:
- Driver’s License, State or School ID
- Social Security Card
- Birth Certificate
- Government Assistance (Food, Cash, SSI Benefit Summary)
- School Schedule or Diploma
- Recent Paystub (Self or Parent if Age 16-17)
STEP 3: Attend an Enrollment Session
Location: 1850 North Perry | Pontiac, MI 48340
Attendees are expected to be on time and stay for the entire meeting. Persons 16-17 years of age must be accompanied by a parent or legal guardian.
